Congenital leukemia (CL) refers to diagnosed at birth or within the 1st month of life. Incidence is reported be 1 in 5 million. We report case a 22-day-old neonate, who presented us with features sepsis, predominant blasts peripheral smear, and was subsequently detected have CL. Although etiology unknown, presence suggests possible intrauterine exposure drugs other toxins genetic abnormalities. The course CL one rapid deterioration death from hemorrhage infection unless treated appropriately, adequate chemo radiotherapy. These neonates pose huge challenge treat due their high mortality, owing both risk treatment-related mortality disease relapse. Early diagnosis prompt initiation treatment are essential for better prognosis.
